유당불내성 검사/ 락토스 검사/ lactose tolerance test (LTT)

만성 설사이다 

사실 언제부터 설사였냐고?  정말 너무 오래전부터라 그냥 살았었다. 내생각엔 20대극초반 인듯? 

모든 의사들이 그냥 IBS 인거 같다고만 하고 끝. 참 정없다. 의지도 없고. 

 

호주에 오고 나서 이게 왠지 lactose intolerance 랑 gluten 문제인거 같았다. 왜냐하면 증상이 글루텐 (밀가루 등) 을 먹거나 우유를 먹으면 너무 심했어서... 그래서 그냥 나는 둘다 문제가 있구나 생각만 하고 살았지, 진단받을 생각은 안했다. 

 

근데. 그냥 한번쯤은 진단 받고 제대로 식사 지키고 살아야 겠다 싶어서 GP 한테가서 증상설명하고 두 검사 진행했는데... 

 

락토즈 톨러런스 테스트 (Lactose tolerance test) 

LACTOSE TOLERANCE TEST

Specimen	2 mL blood in lithium heparin tube prior to, and at 30 minute intervals.
Protocol	The patient takes 1 g lactose per kg body weight up to a maximum of 50 g in a flavoured drink over 10 min. Blood specimens are taken pre-test and half hourly for 2 h. The specimens are analysed for glucose (spectrophotometry). Lactose tolerance can also be assessed by measurement of breath hydrogen every half hour for 3 h after the lactose. See Hydrogen breath. 몸무게 1kg당 락토즈 1g , 최고 50g 음료를 10분동안 마신다 (나도 그랬다)  피검사를 음료 마시기 전, 그리고 매 30분마다 2시간동안 뽑는다. (나는 1.5시간만 하던데..?)  피검사는 당을 측정하게 된다. 락토즈 톨러런스 (유당 내성)은 락토즈 마시고 매30분 마다 3시간동안 수소호기검사(hydrogen breath test) 로도 받을수 있다 (나는 왜 이걸로 안받은거지....................?그럼 피를 4번 안뽑아도 됐었자나...?)
Reference interval	Plasma glucose should increase by at least 1.0 mmol/L in normal individuals. 혈당 레벨은 최소1  mmol/L 정도 올라야 정상이다.
Application	Investigation of lactose intolerance. This test is now rarely performed, as it frequently results in diarrhoea and abdominal pain in affected individuals. It has been largely superseded by the assay of lactase in a small bowel biopsy. See Disaccharidases - small bowel mucosa. 이 검사는 최근는 거의 시행되지 않는데, 그 이유는 이 검사가 유당불내성있는 환자들에게 설사와 복부통증을 유발하기 때문이다. (그래..... 설사와 복부통증이 매 30분마다 있어서 하루 종일 20번 넘게 설사했다.....)  이검사는 소장내시경 조직검사로 락테이즈 (락토즈 분해 흡수도와주는 효소) 확인하는걸로 대체됐다...(나는???.....)
Interpretation	A subnormal rise in plasma glucose indicates lactose intolerance. 혈당이 정상보다 낮게 오르는 경우 유당불내성이라고 진단받게된다 (난 그냥 결과 안봐도 그럴거 같다...)

결론

• 검사받는다고 피검사 4번받고 멍들고, 설사 하루종이 20번넘게 하고 배아팠다... =개고생.
• 이런검사를 받지말고 hydrogen breath test 를 의사에게 부탁하자...........
